Introduction

Thank you for scheduling your appointment with Lincoln Center OBGYN, PA. We are very excited to welcome you into our family of patients. We hope the following will provide you with the information you require about the basic aspects of our clinic.

Fees and Payment

It is our goal to keep the services we provide as affordable as possible for everyone. You can help by paying for your care within 30 days from the date of service or insurance payment. We have Business Office staff available Monday through Friday from 8:30—5:00 at either location to answer your questions and to help with financial arrangements in advance of, and throughout the treatment process.

Insurance/Referrals

Please bring your insurance information with you to your first visit.

Please note that Health Insurance and other benefits such as Medicare, Medicaid, Health Care Savings Accounts (HSAs), etc. may cover some or most of the costs of your treatment(s) but most require that you share in the financial responsibility. You may be required to pay a co-payment, deducible, co-insurance, or other expense(s). Please be prepared to pay your co-payment at the time services are rendered and any other expenses you are personally responsible for within 30 days from the date of service or insurance payment.

Please note that it is your responsibility to obtain referrals from your primary care physician if they are required, and to keep us continually updated with your most current insurance information as well as your personal address and contact information as changes occur.

Lincoln Center accepts assignment with Medicare and Medicaid and has contractual agreements with many of the private insurance plans in our area. We however cannot guarantee participation with every insurance company, plan, or insurance product and because of the complexities involved we encourage you to verify our participation by contacting your insurer directly. We gladly file claims on your behalf for the medical services we provide.

Laboratory, Hospital and Precertification Restrictions and/or Requirements

Please check with your insurance company and make us aware of plan restrictions requiring you to utilize a specific Clinical Laboratory and/or Hospital. If we recommend surgery for you, we also recommend you contact your insurance company to ask if they require precertification or preauthorization prior to surgery.
